We want to make sure you get the most out of your time with your Care Team, so we’re breaking down each appointment type and sharing all the information you’ll need for a successful visit!
NEW PATIENT VISITS
During this appointment, you’ll get to meet your Primary Care Provider (PCP) for the first time! Once introductions are out of the way, you’ll have a chance to review your medical history and discuss any health concerns you may have. Please note, we are not able to prescribe controlled substances in virtual visits.
Goal: To meet your PCP and do a deep dive into your medical history.
PRE-APPOINTMENT CHECKLIST
VIRTUAL VISITS
- Set The Environment
- For this appointment, it’s important that you’re located in a private, well-lit place with a stable internet connection. It’s also important that you’re not driving and that you’re located in the same state you were in when you booked the appointment. If you plan on being in a different state, let us know via chat so we can set you up with a PCP licensed in that state.
- Come Prepared: Please come ready to discuss the following:
- Medical history (past medical/surgical history, any new allergies, family history, etc.)
- Medical records
- Current medications
- Vitals including blood pressure, heart rate, height, and weight
- Upload Your Insurance Information
- If you haven’t already uploaded your insurance, go to the “Profile” tab in the Eden Health app and tap “Insurance” to add photos of the front and back of your card. This step must be completed prior to your appointment to avoid incorrect billing.
- Let Us Know If You Can’t Make It
- We get it, life happens! If you need to cancel or reschedule your appointment, you can do so in the Eden Health app. Go to the “Appointments” tab, and you’ll see all upcoming (and past) appointments listed. We request that you reschedule or cancel 24 hours before your start time so that appointment time can become available for others.
IN-PERSON VISITS
- Come Prepared: Please arrive 5 minutes before your appointment start time, and check-in on the iPad Kiosk. For your visit, please come ready to discuss the following:
- Medical history (past medical/surgical history, any new allergies, family history etc)
- Medical records
- Current medications
- Vitals including blood pressure, heart rate, height, and weight
- Upload Your Insurance Information
- If you haven’t already uploaded your insurance, go to the “Profile” tab in the Eden Health app and tap “Insurance” to add photos of the front and back of your card before your appointment. ANNUAL WELLNESS VISITS
Your Annual Wellness Visit is an opportunity for you and your PCP to review your wellness and come up with a personalized prevention plan.
From specifics like risk factors influenced by your family history to your overall physical and mental well-being, your Care Team is ready to help.
Goal: To catch potential health issues early! We want to catch issues before they become serious so you can focus on wellness and a healthy lifestyle. This visit may include important screening tests, vaccinations, and other recommended testing based on your history. Please note, we are not able to prescribe controlled substances in virtual visits.

FOCUSED VISITS
Focused visits are a chance for you and your provider to discuss any specific health concerns that may arise. These include, but are not limited to high-blood pressure, pain, allergy or respiratory issues, anxiety or depression, weight management, or smoking cessation. Please note, we are not able to prescribe controlled substances in virtual visits.
Goal: To follow up on your personalized Care Plan, to address any acute health issues you’re experiencing, and to manage chronic conditions if appropriate.
